Besides courses and documents, the Training Centre can ask you to fill in forms: an induction checklist, a self-assessment, a record of on-the-job training. There are two kinds. Assigned forms are ones a supervisor has given you to complete, and wait for you on the home screen. Forms you start yourself are form templates your company has opened to trainees, which you fill in whenever you need to, for example each time you do a task that has to be recorded.

Where to find it: Training Centre > Forms > Complete Form for an assigned form, or Training Centre > ··· (more) > Forms for forms you start yourself.

Filling in a form

Forms are built by your company, so their fields differ, but every form works the same way:

Control What it does
Camera icon (top right)Take Photo: attach a photo to the form, for example of a completed check. Hidden once the form has been confirmed.
SaveKeeps what you have entered. Leave with the back arrow and carry on later.
ConfirmSubmits the form. Required fields must be filled in first; if one is missing, a message names it: “field name” must be entered before confirmation. Confirm is hidden while your device is offline.
Back arrow (top right)Leaves the form. Anything not saved is lost.

After Confirm you return to the previous screen. A confirmed form can no longer be changed from the portal.

Availability and permissions

  • Assigned forms come from the trainee form templates your training manager maintains under Setup > Training > Trainee Form Templates, and are assigned from your trainee record with Add Form.
  • Forms you start yourself are form templates whose Roles with Access setting includes Trainee, at your site. If the list is empty, none have been opened to trainees.
  • You see and edit only your own forms.

Things to know

  • Only assigned forms appear in Archives. Completed forms you started yourself are not listed in the portal; staff can see them in QTRACA.
  • Save before you leave. The back arrow does not warn you about unsaved changes.
  • Forms are not courses. Filling in a form does not complete a training course or a learning path step.
